Monolog Desktop Extensions

Desktop notifications extensions (handlers / formatters) : notify-send (Linux) and Growl (Mac OS) for Monolog PHP library.

These handlers are heavily inspired by Fabien Potencier's [Sismo](https://github.com/fabpot/Sismo) application's notifiers.
